Accountability: The Direct Support Personnel (DSP) shall be accountable to the Team Lead.

Duties: The DSP will:

• Implement risk plans correctly and safely
• Assist individuals served with activities of daily living (personal hygiene, grooming, feeding, toileting, cooking, cleaning)
• Maintain a healthy and safe environment
• Assist individuals with goals
• Use adaptive equipment correctly
• Administer medication(s) according to guidelines.
• Implementing treatment plan within scope of practice
• Encourage community participation of individuals served
• Communicate effectively with team members
• Document accurately and timely
• Monitor for changes in health and behavior and communicate to support team
• Assists individuals in performing their cleaning responsibilities or/and perform cleaning duties as applicable.
• Transport and accompany individuals to appointments, outings, and shopping.
• Perform other responsibilities and duties assigned by supervisor.

Qualifications: High school diploma or equivalent. Preferred previous experience with the developmentally and intellectually disabled preferred.
