Residential Habilitation and Support (RHS) services are individually tailored supports that are specified in the
Person-Centered Individualized Support Plan (PCISP) to assist with the acquisition, retention, or improvement in skills related to living in the community. Our RHS services are focused on ensuring that individuals have good health, safe living environment, and are able to keep up with their skills and learn new skills. RHS services are provided in the residences of the individuals served. RHS services can be provided 24 hours a day, 7 days a week, and 365 days a year depending on the needs of individuals served
RHS services are available only under the CIH waiver.
We provide support in the following life skill-development and community engagement areas including but not limited to:
- Staff supervision and monitoring
- Assistance with personal care
- Assistance with ADL
- Coordination of health care needs
- Medication monitoring/management
- Behavior management and monitoring
- Skills training
- Assistance with meals
- Transportation
- Group activities
- Community integration (recreation, shopping, etc.)
- Financial management
