Day Habilitation

Day habilitation are services that are specified in the Person-Centered Individualized Support Plan (PCISP) and support learning and assistance in the areas of: self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Day habilitation activities are intended to develop skills related to their abilities and interests; build relationships and natural supports; and foster independence. Services are provided in a variety of settings in the community or in a facility owned or operated by an FSSA/DDRS-approved provider. Settings are non- residential and separate from an individual’s private residence or other residential living arrangements.

Day habilitation services are available under the FSW and CIH waiver.

Day activity choices include but are not limited to:

  • Health and fitness
  • Recreational activities (Bingo, etc.)
  • Vocational activities (Arts, gardening, technology)
  • Volunteer opportunities in the community
  • Sports activities
